What are your current operating hours and location?

For now, our operating hours will be Mondays and Tuesdays from 6PM-7PM and our location is at 300 East 2nd Street, Suite C, Muscle Shoals, AL.  The location corresponds to Drip Lyfe IV Hydration.  Our Medical Cannabis Certification Clinic will be sharing space with Drip Lyfe for now and use their facility afterhours only when Drip Lyfe is normally closed.

What do I need to bring to my certification appointment?

Bring proof of Alabama residency through a valid photo ID.  Also bring any controlled substance prescriptions that you receive from a physician.

What happens during my certification appointment?

For your first appointment, you will fill out paperwork summarizing your medical history and current medications prescribed.  You will then provide a urine drug screen and meet with our certifying physician.  He will discuss your medical reason for seeking medical cannabis and make a determination if you would qualify for certification.

What would disqualify me from receiving a medical cannabis certification?

Not having a qualifying medical condition or having a medical history in which THC use would be considered detrimental would disqualify a patient from receiving certification.  In addition, a urine drug screen that contains illicit drugs (such as methamphetamines, cocaine, opiates or benzos that are not prescribed).  THC in your urine drug screen does NOT disqualify you.   Finally, if you are a pregnant, breastfeeding or a woman attempting to conceive you are not eligible for medical cannabis certification.

What is the minimum age for receiving a medical cannabis certification?

19 years of age.  While someone that is younger than 19 can technically receive medical cannabis certification in Alabama if they have a registered caregiver, we do not do certifications to those younger than 19 at Leaf Life Medical Cannabis Certification.

What happens after my medical cannabis certification visit with the physician?

Your certifying physician will complete an online application in the Alabama Medical Cannabis Commission portal certifying you as being recommended to receive medical cannabis.  You will receive an email from the Alabama Medical Cannabis Commission requiring you to register in their patient portal and provide information and a registration fee.  The Commission then finalizes your approval and you should be able to retrieve your card through the portal.

If you do not receive an email from the Alabama Medical Cannabis Commission the day after you have performed your physician evaluation for certification, you can go to the Alabama Medical Cannabis Commission portal at  https://al-amcc-public.nls.egov.com/login.  You will first need to register a new account on the portal.  You then create a new application as a patient for certification and follow the prompts for completing the application, which involves uploading a photo of your Alabama ID.

An additional $45 fee is charged by the Alabama Medical Cannabis Commission portal for completion of certification application.

How long is Alabama Medical Cannabis certification good for?

Except for women with child-bearing potential, your medical cannabis certification is approved for 90 days.  After that you must renew with a medical cannabis certifying physician to continue receiving medical cannabis.  For women who are capable of having children, the certification is good for 30 days and must be renewed every thirty days.

Can I certify or recertify for medical cannabis through telehealth in Alabama?

No.  As of now, certifying physicians in Alabama must have an in-person visit with patients seeking certification or recertification.

What are the costs associated with medical cannabis certification?

The cost of initial certification across all patients is $200.  The cost of any recertification or follow up visit is $100 for all individuals incapable of conceiving children.  Because women capable of bearing children must certify every 30 days, the cost of each recertification is $35.

Is health insurance accepted for certification costs?

We are not accepting any health insurance for certification costs.

What is the THC dose limit for medical cannabis in Alabama?

The highest initial daily approved THC dose is 50 mg daily.  This can be adjusted to 75mg daily only after having attempted 50 mg daily and not having sufficient relief of symptoms while not causing adverse effects as determined by the certifying physician.  Terminally ill patients only may qualify for higher doses than that but must surrender the ability to drive through suspension of their driver's license.

No, not for medical cannabis certification needs.  The facility that we do our medical cannabis certification normally functions as an IV hydration clinic during the day.  The staff at the IV hydration clinic there will be unable to help you.  We do our medical cannabis certification physician visits afterhours at this location on very specific days and times, currently 6-7PM on Mondays and Tuesdays.  You may walk-in during those specific hours but nobody will be available there to help you during regular day hours when the IV hydration clinic is operating.  If you need to reach out to us, you can call us at 256-460-4131 or email us at leaflifealabama@gmail.com.
